Findings: The facility failed to develop, initiate, and implement person-centered interventions to keep cigarettes and lighters in a secure place and out of Alleged Victim's reach. AV has an ongoing history of smoking in his/her apartment and other areas inside the facility which placed AV and others at risk of harm. The facility's failure to implement interventions and appropriately care plan related to AV's behaviors is a violation of resident rights, is considered neglect of care and constitutes abuse.
