Findings: Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim’s (AV) care plan to speak softly and never pull AV when walking, walk at his/her pace, and if AV pulls away, let go and provide gentle direction. On or about April 26, 2022, AP2 was heard yelling at AV and then AP2 was observed physically pulling AV up by the arm and then using AP2 body weight to push AV forward in what appeared to be an attempt to force AV to move, causing AV loss of dignity and putting AV at risk for harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from rough treatment which is a failure of Oregon Administrative Rules.
