Findings: On or about March 4, 2022, morning staff arrived to find the Alleged Victim (AV) in his/her recliner, where he/she had been left the evening before. AV had a soaked brief and had not been changed during night shift. Alleged Perpetrator #2 (AP2) worked night shift, and according to video documentation, AP2 never approached AV to provide care, and in fact, AP2 was seen on video with his/her head down on the table sleeping. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
Sanction: RCFCP22-00953 $500.00 fine assessed
