Findings: On or about May 2, 2022, the Alleged Victim (AV) had a unwitnessed fall resulting in a head injury, and AV was sent to the hospital for treatment two days later, on May 4, 2022. AV’s Service Plan indicates AV is a known fall risk, uses assistive devices (walker and wheelchair), requires assistance with transfers and ambulation escorts, but also states AV is independent with ambulation. The facility failed to implement interventions and care plan to appropriately address AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01325 $375.00 fine assessed
