- Findings
- The facility failed to implement interventions and appropriately care plan related to the AV’s fall history, need for skin monitoring and directions on what to do if W2 refuses care. The failure resulted in AV experiencing falls with injury and risk of ongoing harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
