Findings: On or about March 31, 2022, the Alleged Victim (AV) suffered a fall in his/her room, causing an injury to his/her head. AV was sent to the hospital and returned with 5 staples to close the laceration to AV's head. The facility failed to implement interventions to ensure AV's safety regarding falls. The facilities failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
