Findings: The Alleged Victim (AV) has had multiple falls at the facility. On or about April 17, 2022, AV suffered a fall while trying to pull up his/her pants, causing an injury to his/her head. AV was sent to the hospital and returned with steri stips close the laceration to AV's head. The facility failed to implement interventions to ensure AV's safety regarding falls. The facilities failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
