Findings: According to documentation, the AP2 failed to provide a safe medication administration system when s/he gave AV another resident's medication. The failure resulted in AV being transported to the hospital in a sedated state, which is a violation of resident rights, is considered neglect of care which constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
