Findings: Alleged Victim (AV) has a known history of balls. Between April 10th, through May 8th, 2022, AV sustained five (5) unwitnessed falls, two resulting in injury. On or about May 8, 2022, staff heard a noise and found AV on the ground near the wall. Staff found blood on the A/C unit where AV was lying near, and AV had a “small pool of blood” emanating from his/her mouth and a tooth was found in the blood. The facility failed to provide a safe environment, which resulted in AV sustaining falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP22-01193 $500.00 fine assessed
