Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about April 3, 2022, and again on or about May 10, 2022, AV was found outside the locked facility doors. AV has a history of exit seeking and eloping, and despite a previous elopement on or about February 8, 2022, where AV was found approximately half mile away from the facility at night, no additional interventions were put in place to prevent this from happening in the future. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01284 $500.00 fine assessed
