Findings: Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim's care plan to be woken up twice per night for incontinence care. At time of incident, AV was discovered wet and that it went through AV's mattress cover and sheets. AP2 is responsible for neglect of care which constitutes abuse. The facility failed to ensure AV's care plan was followed at time of incident which is a violation of Oregon Administrative Rules.
