Findings: The Alleged Victim (AV) is known to have exit seeking behavior and there was a known mechanical issue with the door that was not fixed in a timely manner. On or about May 11, 2022, AV was found outside the facility, which exposed AV to potential harm. The facility failed to provide a safe environment for AV which is a violation of resident rights, is neglect of care which constitutes abuse.
