Findings: Alleged Victim (AV) is scheduled to receive eye drops 3x a day at 8am, 11:30am and 8pm. Between May 1st through May 18th, 2022, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) provided AV with his/her eye drops. AP2 provided AV’s eye drops a total of six (6) times and AP3 provided AV’s eyes drops a total of 12 times. Both AP2 and AP3 gave AV the eye drops between one (1) hour and up to three (3) hours past 8am. AP2 and AP3 failed to provide a safe medication administration system for AV, which constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administration Rules.
