Findings: The Alleged Victim (AV) relies on the facility for care and safety. AV’s care plan states, AV is to use his/her walker at all times while ambulating, facility staff will assist AV with dressing, and address any observed safety hazards. The facility was aware AV’s room contained furniture that did not allow AV to maneuver h/h walker in some areas. On or about May 8, 2022, AV fell while attempting to get dressed without staff present, and in an area that AV’s walker did not fit, resulting in AV being hospitalized with a broken hip. The facility failed to follow AV’s care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00314 $500.00 fine assessed
