Findings: On or about June 2, 2022, the Alleged Victim (AV) was found on the floor at 7:20 am. AV was soaking wet and had fallen due to being soaked and not changed during the night. A staff member did not show up for his/her shift, leaving residents unchanged all night. AV was care planned to have safety checks every thirty minutes and toilet at 1:00 am or if AV is awake. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
