Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)’s needs, resulting in no one responding to AV call light timely after AV fell and hurt h/h knee sustaining an abrasion and swelling. Documentation provided to the department shows AV on or about May 22, 2022, fell and pressed h/h call button. The call light was not answered, AV scooted h/h-self out to hallway and used the hallway handrails to get h/h-self up. AV walked to the office and notified staff of the fall. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegations Alleged Perpetrator allegedly failed to answer the call light in a timely manner for the Alleged Victim was investigated and determined no licensing violation or abuse occurred.
