Findings: The facility, and Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system to ensure the Alleged Victim's (AV) medication was administered as ordered. On or about May 24, 2022, AV received two doses of medication, placing AV at risk of harm. The facility, and AP2’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
