Findings: The facility failed to assess, intervene, and monitor the Alleged Victim (AV) when he/she experienced a change of condition. On or about, May 26, 2022, the AV was found outside the assisted living facility. Prior to the incident, AV was exhibiting increased wandering within the facility and experiencing cognitive decline. The facility's failure placed AV at serious risk of harm, which is a violation to resident rights, is considered neglect of care and constitutes abuse.
