Findings: On or about May 13, 2022, the Alleged Victim (AV) was having behaviors. Alleged Perpetrator #2 (AP2) grabbed AV by the wrists to restrain him/her, causing a bruise. This is not a technique that staff were trained to use to redirect AV. AV was care planned for behavioral interventions at the time of the incident. AP2's actions are a violation of resident rights, are considered neglect of care and constitute wrongful restraint. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
