Findings: On or about May 26, 2022, the Alleged Victim (AV) suffered a scratch to his/her forehead by scratching him/herself. AV's fingernails had not been cut for 3-4 months, even though family had repeatedly requested the nails to be cut. The facility's failure to provide service for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
