Findings: Alleged Victim (AV) relies on the facility for his/her care. AV recent needs had changed, including increased urinary incontinence, increased assistance with brief changes, AV refusing to have sheets changed and refusing showers. AV was also found numerous times to be laying on a soiled bed pad or mattress and in soiled briefs. The facility did not have documentation that the notifications occurred or that the facility responded to the refusal of showers or resistance to care. No additional interventions were put in place and on or about May 27, 2022, AV was diagnosed with a pressure ulcer. The facility failed to assist AV with his/her hygiene and failed to care plan in accordance with AV's capabilities which is a violation of resident rights is neglect of care and constitutes abuse.
