Findings: The facility failed to protect the Alleged Victim (AV) from mental and emotional abuse. Witness #1 (W1) and Witness #2 (W2) engaged in multiple altercations, making AV fearful for W2, causing AV to try to protect W2 from W1. AV was alarmed by W1 harming W2 to the extent that AV wants to move from the facility. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
