Findings: According to the documentation, an unknown perpetrator (AP3) failed to follow the care plan of the Alleged Victim (AV) by not placing their fall mat next to their bed when the AV was asleep. The failure resulted in the AV suffering a fall causing an abrasion to their scalp, which is a violation of resident rights and is considered abuse. The facility failed to follow the care plan which is a violation of Oregon Administrative Rules.
