Findings: Alleged Victim (AV) relies on the facility for his/her care. Concerns are reported regarding AV not receiving care in a timely manner after using call pendent. Call logs for the month of May show approximately nine (9) response times exceeding an hour. On or about May 31, 2022, AV pushed call light for assistance to the bathroom and did not have a timely response which resulted in AV having an incontinence episode in h/h wheelchair causing loss of dignity. The facility failure to answer call lights in a timely manner have resulted in AVs care needs not being met, which is a violation of resident rights, is neglect of care and constitutes abuse.
