Findings: The facility failed to provide a safe medication administration system to ensure The Alleged Victim’s (AV) medications were administered as ordered. Based on interviews and facility documentation, it was determined that AV did not receive his/her psychotropic injection ever 3 weeks as ordered since December 2021, causing unreasonable discomfort. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
