Findings: The facility failed to implement interventions or appropriately care plan for the Alleged Victim’s (AV) known behaviors. This failure resulted in an altercation on or about June 2, 2022, between AV and Witness #1 (W1), where AV received skin tears and bruising to AV’s hands and arm, which caused unreasonable discomfort. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse
