Findings: The Alleged Victim (AV) had a history of wandering, getting lost. On or about June 01, 2022, he/she eloped from the facility campus, and was located by a security guard at the hospital AV wandered to. The facility failed to appropriately care plan and failed to provide a safe environment for AV. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
