Findings: The facility failed to follow the care plan. AV is care planned to use a gait belt for all transfers. On or about June 01, 2022, it was discovered AV had bruising on both arms from being transferred without a gait belt, which is a violation of resident rights is neglect of care and constitutes abuse.
