Findings: On or about June 4, 2022, Witness #4 (W4) entered the Alleged Victim's (AV) room and was observed to have his/her genitalia out of his/her pants and was fondling AV's chest. Staff removed W4 from AV's room, however, later W4 was again in AV's room doing the same thing as he/she was found doing earlier in the day. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01338 $338.00 fine assessed
