Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about June 5, 2022, staff did not lock the door to an office which caused AV to elope the facility when they were not watching. AV left the facility without shoes on and walked near a busy street. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
