Findings: Alleged Victim (AV) and Witness 1 (W1) have history of verbal altercations and aggressive behavior with other residents. On or about June 06, 2022, AV and W1 behaviors escalated from verbal incident to physical incident resulting in both falling to the ground. The facility failed to appropriately adjust care plans and implement reasonable interventions to address AVs and W1s continued aggressive behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
