Findings: Alleged Victim (AV) is care planned to be unable to ambulate safely and independently and requires a 2-person assist to sit and stand-up and requires the use of a walker at all times with stand-by assist. On or about June 7, 2022, AV wandered two blocks away from the facility by him/herself. As a result of the elopement, AV fell and scrapped his/her elbow and had to be brough back to the facility by police. The facility failed to follow the care plan around AV’ ambulating requirements when AV eloped from the facility by themselves, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP24-00060 $250.00 fine assessed
