Findings: On or about June 13, 2022, Witness #1 (W1) hit the Alleged Victim (AV) on the head with a remote, causing a red mark to AV's head. W1 is a known aggressor in altercations, and has interim service plans in place to have resident in place where staff can see him/her and frequent checks, however, there is nothing to state how frequent checks should be. The facility did not have enough staff present to ensure this resident could be in sight of staff. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
