Findings: The Alleged Victim (AV) is care planned for one person assist with transfers and has a history of falls, several related to self-transferring without assistance. AV had approximately 22 falls between March 2022, and May 2022, resulting in injuries, including bruises and abrasions to his/her back, abrasions to knees, redness to forehead, swollen ankle, and bruising on hand. The facility failed to properly care plan to mitigate risk of falls and self-transferring. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP23-01075 $500.00 fine assessed
