Findings: According to the documentation, the facility failed to care plan for Witness 1’s (W1) known behaviors towards other residents. The failure resulted in W1 getting into an altercation with the Alleged Victim (AV) causing the AV to fall, hit their head and received a laceration, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
