- Findings
- Alleged Victim (AV) has a history of wandering around the facility and entering other residents’ rooms. On or about June 20, 2022, AV wandered into Witness 1 (W1) room, W1 reported she/he tried to get AV out and AV fell and hit head causing a cut above AVs eyebrow. The facility failed to properly plan care and provided interventions appropriate to mitigate AV ongoing wandering, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
