Findings: Witness #1 (W1) has a history of inappropriate behaviors with other residents and wandering around the facility. Alleged Victim (AV) has a history of leading other residents into h/h bed. On or about June 23, 2022, W1 wandered into Alleged AVs room and were found together in AVs bed engaging in inappropriate behaviors. The facility’s failure to follow W1s care plan and to properly plan care and provide interventions appropriate to mitigate AVs inappropriate behaviors, placed both residents at risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
