Findings: The Alleged Victim relies on the facility for his or her safety. On or about June 26, 2022, AV was attending a mass service, but did not wait for a caregiver to escort h/h back to the facility. According to documentation, AV’s wander guard bracelet had been removed by unknown means, so h/s was able to go out the door without it automatically locking. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00287 $188.00 fine assessed
