Findings: The Alleged Victim (AV) is known to spend a lot of time on the porch and to walk around barefoot. According to an investigation, on or about June 26, 2023, AV was found on the porch with significant burns on both feet and transported to the hospital for treatment. It was discovered that AV had been on the porch barefoot for several hours, in approximately 98-degree weather. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
