Findings: AV has a history of frequent infections and skin breakdowns, and is care planned for two showers a week. According to an investigation, there were only three documented showers for AV in the month of June 2022, due to lack of staff. AV experienced unreasonable discomfort and a loss of personal dignity. The facility failed to follow AV's care plan, which is a violation of resident's rights, is considered neglect of care and constitutes abuse.
