Findings: The facility failed to implement interventions or appropriately care plan for the Witness #1’s (W1) known behaviors. This failure resulted in an altercation on or about June 29, 2022, between W1 and the Alleged Victim (AV), where AV was pushed into a wall and received a skin tear on their arm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
