Findings: On or about April 26, 2022, the Alleged Victim's (AV) medication was inadvertently discontinued in error. On or about June 21,2022, it was discovered that the medication had been discontinued, as there was a computer glitch while making an update to the medication on AV's record. AV did not receive needed medication for approximately two months, placing AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
