Findings: Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system. On or about June 30, 2022, AP2 administered another resident a cup of coffee mixed with medication. AP2 failed to watch the other resident completely drink the coffee mixed with medication and walked away. The other resident gave Alleged Victim (AV) the coffee cup and AV is seen on video footage drinking from the cup. AV was transported to the hospital as a result of not feeling good after drinking the coffee mixed with mediation. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
