Findings: Alleged Victim (AV) relies on the facility for a safe environment. AV has a history of unknown bruising. On or about July 04, 2022, AV was found to have a bruise on h/h left and right forearm and left hand by thumb. The facility failed to implement interventions for AV ongoing bruising and failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
