Findings: On or about June 6, 2022, the Alleged Victim (AV) wandered into the kitchen and went to a small sink. Alleged Perpetrator #2 (AP2) was in the kitchen and told AV to leave. AV refused to leave, AP2 started yelling at AP2. Staff overheard and came into the kitchen to try to get AV to leave, however, AV didn't. AP2 determined to get AV out of the kitchen by twisting AV's arm behind his/her back and forcing AV out of the kitchen. During this action, AV was refusing by dragging his/her feet. It was discovered later that AV had suffered some scratches from the incident. AP2's actions are a violation of resident rights, are considered neglect of care and constitutes physical abuse. The facility's failure is a violation of Oregon Administrative Rules.
