Findings: Alleged Victim (AV) AV has a long history of violent behaviors with staff and other residents. AV requires extensive staff assistance with reminders, supervision, and redirection. AV is care planned with interventions that include one-on-one. On or about July 07, 2022, AV took utensils from dining room to use as weapons against h/h-self and others and broke a window. The facility failed to follow the care plan and provide one-on-one, due to staff shortage, which is a violation of resident rights is neglect of care and constitutes abuse.
