Findings: The facility failed to follow the care plan and provide appropriate services according to the Alleged Victim (AV)'s needs relating to ambulation. AV is a high fall risk and is care planned for staff to escort AV to meals and provide stand by assistance with mobility and help AV sit down in h/h chair at the dining room table. On or about July 09, 2022, AV was found sitting on the floor next to h/h chair in the dining room, AV reported trying to sit down and missed the chair resulting in AV sustaining a bump and redness over h/h left eyebrow/temple. This failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
