Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victims (AV) medication orders were administered as prescribed. On or about June 26, 2022, and June 27, 2022, AV missed prescribed medication, and was involved in a Resident-to-Resident altercation on or about June 27, 2022, causing risk of serious harm. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
