Findings: Alleged Victim (AV) relies on the facility for his/her care. Between April 19th through July 8th, 2022, AV had 13 documented falls in the facility. On or about July 8, 2022, AV sustained a fall in his/her room that resulted in a fracture in AV’s hip. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
