Findings: The Alleged Victim (AV) is care planned to be checked by staff every two hours. On or about July 12, 2022, AV was found in his/her room, naked, cold and soiled with feces and dripping blood from his/her genital area. Alleged Perpetrator #2 (AP2) had been on shift, with AV in his/her care area prior to AV being found. AV's condition would have taken more than two hours to have developed. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
